Core and Capacity Building

At Heroes Assist, our support work services are delivered under both Core Supports and Capacity Building Supports, as outlined in the NDIS framework. We focus on building independence, promoting inclusion and helping participants reach their individual goals through consistent, person centred care.

Services under Core Supports – Assistance with Daily Life:

• Personal care including showering, dressing, grooming and mobility
• Support with household tasks such as cleaning, laundry and meal preparation
• Assistance with daily routines, including prompts for medication and budgeting
• Transport to appointments, community access, and social engagement
• Support to maintain a safe and comfortable home environment

Services under Capacity Building Supports – Increased Social and Community Participation & Improved Daily Living:

• Skill development in areas such as cooking, shopping, planning and household management
• Building confidence in social settings and community participation
• Supporting participants to access and engage in education, volunteering or work pathways
• Promoting independence through goal setting, emotional regulation and routine building
• Strengthening life skills such as communication, personal organisation and travel training

Our team is experienced, respectful and trauma informed. We work closely with participants, their families and service providers to ensure all supports align with individual NDIS goals and plan outcomes. Whether the focus is on maintaining daily function or building new skills, we are committed to supporting each person’s growth, independence and wellbeing.
