Navigating the NDIS can be complex, but Heroes Assist is here to support you every step of the way. Our Capacity Building Supports, delivered through Support Coordination, are designed to help you understand your plan, connect with the right services, and stay on track with your goals.

Support Coordination includes:

Plan Understanding and Budgeting

Help interpreting your NDIS plan and funding categories
Guidance on using your budget effectively to meet your goals
Support with planning, tracking, and adjusting spending as needed

Service Connections and Advocacy

Identifying the right services and providers for your needs
Coordinating referrals and service bookings
Advocating to ensure your supports are appropriate, timely and person centred

Ongoing Coordination and Monitoring

Regular check ins to review progress and service quality
Adjusting services and supports as your needs evolve
Problem solving and assistance with service related issues

Our goal is to empower you to make informed decisions, use your plan to its full potential, and stay in control of your support journey.
